I am attempting to create an account to get an eVOA and it asks for “mothers name”. Is this mothers full name or maiden name?

It's not stipulated. It's worth saying that the amount of time and effort it seems to take to do the visa online is much more than it takes to do it at the airport - mother's name is not needed at the airport, nor uploading of photos and digital scans of your passport; all you do is hand over your passport, boarding pass, pose for a photo, get your fingerprints digitally scanned, and then hand over the money. I've never had that process take more than 2 minutes, and the queue wait has never been more than a few minutes.

I put my mother’s maiden name and it was accepted.
#3, You do need your passport when you buy Visa On Arrival (Arrivals Terminal in Bali) before you join the Immigration queues.
Your passport, Visa On Arrival receipt/s, boarding pass, look at camera, index fingers on pad for scanning are required at Immigration.
Hi I have just come across this post as I have the same query. Did you put your mother's full name (first name and maiden name) or just the maiden name please? It only says 'mothers name' so not clear if it's her full name or just her maiden name. Thanks
What is it you are trying to do? If you are applying for the 30 day evoa you do not need to create an account, and you certainly dont need to input parent names. https://evisa.imigrasi.go.id/web/application_voa

Whatever issues the OP was faced with trying to create an acct, that was 2 years ago when the Indonesia Minister for Website Glitches didn't quite have their act together.
As you know, the E-VOA process has mercifully been simplified since then.
It still asks for this today when I am filling in the form! Does anyone know?
